What is paediatric nephrology?

Paediatric nephrology is the medical specialty focused on the diagnosis, treatment, and follow-up of conditions affecting the kidneys and urinary system in babies, children, and adolescents.

The kidneys play a vital role in maintaining the body’s balance by regulating the elimination of fluids, salts, and toxins. When their function is altered, symptoms may appear such as recurrent urinary tract infections, blood in the urine, difficulties with urination, or changes in blood pressure.

Paediatric nephrology treatments at home

Repeated episodes of urinary tract infection that require thorough evaluation to identify underlying risk factors (such as vesicoureteral reflux or voiding dysfunction) and to establish a long-term prevention and treatment strategy to protect kidney function.

This includes a spectrum of structural anomalies present from birth, such as dysplastic kidneys, renal agenesis, duplication of the collecting system, or abnormalities of the ureteropelvic or vesicoureteral junction. Management ranges from careful monitoring to surgical intervention.

Voiding disorders, or bladder dysfunction, include difficulties related to the storage or emptying of urine, presenting as urgency, increased frequency, daytime incontinence, or incomplete emptying.

Nocturnal enuresis, commonly known as bedwetting, refers to involuntary urination during sleep beyond the expected age for bladder control. Management involves an initial evaluation to rule out underlying medical causes, followed by behavioural therapy and, in some cases, pharmacological treatment.

Conditions affecting the glomeruli, the kidney’s filtering units, such as nephrotic syndrome or glomerulonephritis.

Widening or swelling of one or both urinary tracts or the kidney, often detected prenatally or in early childhood. It may be caused by obstruction or reflux, and careful follow-up is essential to prevent progressive kidney damage.

The presence of red blood cells in the urine may be macroscopic (visible to the naked eye) or microscopic. It requires a thorough evaluation to determine its origin (glomerular or non-glomerular) and to rule out serious causes such as stones, infections, or glomerular diseases.

The formation of “stones” in the kidney or urinary tract. Although less common than in adults, their incidence is increasing in children. Management focuses on pain relief, facilitating stone passage, and, importantly, preventing recurrence through metabolic evaluation.

A persistent elevation in blood pressure above normal values for a child’s age, sex, and height. In childhood, hypertension is often secondary in origin, with kidney disease (parenchymal or renovascular) being one of the most common causes. Early detection and appropriate management are essential to prevent future cardiovascular complications.

Which kidney conditions can be managed at home?
